What is chiropractic crepitus (joint cavitation)?

Chiropractic crepitus, or joint cavitation, is the audible cracking or popping sound produced when gas bubbles trapped in synovial fluid are released during a spinal adjustment, caused by a sudden drop in joint pressure rather than bone fracture.

That distinctive popping or cracking sound heard during a chiropractic adjustment is joint cavitation, technically called crepitus. It occurs when the chiropractor applies a controlled, high-velocity thrust to a spinal segment, suddenly reducing the pressure inside the synovial joint capsule. The synovial fluid surrounding the joint vertebrae contains dissolved gases-primarily nitrogen, oxygen, and carbon dioxide. When pressure drops sharply, these gases form bubbles and collapse, creating the audible crack.

The sound itself is not bones breaking or joints misaligning. The actual joint movement is measured in millimeters, and the cavitation is simply a byproduct of the mechanical adjustment. Some adjustments produce audible cavitation, while others do not, depending on joint condition, synovial fluid state, and technique. The bubbles that form typically take 15 to 20 minutes to reabsorb into the fluid, which is why repeated adjustments to the same joint space within minutes usually produce no second crack.

Understanding crepitus matters for patients receiving adjustments in the Klang Valley and beyond. The sound may seem alarming to first-time visitors, but it is not a sign of danger or structural damage. Patients who do not hear cavitation during treatment are not receiving less effective care. Chiropractors monitor joint mobility and patient response to adjustments through palpation, range of motion tests, and symptom changes, not by the presence or absence of sound.
